ACNE: OVERVIEW
Acne is a skin condition that occurs when your hair follicles become plugged with oil and dead skin cells. It often causes whiteheads, blackheads or pimples, and usually appears on the face, forehead, chest, upper back and shoulders. Acne is most common among teenagers, though it affects people of all ages.

Today, effective treatments are available, but acne can be persistent. The pimples and bumps heal slowly or  may continues to grow in size if not well taken care. Even if one begins to go away, others seem to crop up. Acne can cause emotional distress or depression and scar the skin, depending on its severity. Treating acne before cysts and nodules appear can prevent scars. The earlier you start treatment, the lower your risk of such problems.


ACNE SIGNS
Acne can result in several different types of skin lesions. 
Lesions not associated with inflammation include: 
  • Blackheads (also called open comedones) – open pores clogged with sebum (skin oil) and dead skin cells; and 
  • Whiteheads (also called closed comedones) – closed pores clogged with sebum and dead skin cells. 
Inflammatory lesions include: 
  • Papules – small, tender, red skin bumps; 
  • Pimples (also called pustules) – small, tender, red skin bumps with pus at the tip; 
  • Nodules – painful, hard lumps beneath the skin’s surface; and 
  • Cysts – inflamed, painful, pus-filled lumps beneath the skin’s surface due to inflammation deep in the hair follicle. Cysts are very large, soft, painful, red or white lumps situated deep in the skin. They are filled with pus. Cysts form deeper within the skin than nodules, and they are the most severe type of acne blemish. Cysts can also cause skin complications, such as scarring.

HOW DOES ACNE FORM?



CAUSE OF ACNE
The exact cause of acne is unknown, although, several risk factors have been identified:

  • Age. Due to the hormonal changes they experience, teenagers are more likely to develop acne.
  • Gender. Boys have more severe acne and develop it more often than girls. 
  • Disease. Hormonal disorders can complicate acne in girls.
  • Hormonal changes. Acne can flare up before menstruation, during pregnancy, and menopause.
  • Drugs. Acne can be a side effect of drugs including tranquilizers, antidepressants, antibiotics, oral contraceptives, and anabolic steroids. 
  • Personal hygiene. Abrasive soaps, hard scrubbing, or picking at pimples will make them worse.
  • Environment. Exposure to oils and greases, polluted air, and sweating in hot weather aggravate acne.

ACNE SYMPTOMS
Acne can cause more than blemishes. Studies show that people who have acne can have: 
ACNE: LOW SELF-ESTEEM & DEPRESSION
  • Low self-esteem: Most people say that their acne makes them feel bad and ugly. They do not want to be with friends and skip their school or work. Grades can slide, and absenteeism can become a problem because of their acne. 
  • Depression: Some suffer more than low self-esteem that lead to a medical condition called depression. Many studies have found that teens who believe that they have “bad” acne were likely to think about committing suicide. 
  • Dark spots on the skin: These spots appear when the acne heals. It can take months or years for dark spots to disappear. 
  • Scars (permanent): People who get acne cysts and nodules often see scars when the acne clears. You can prevent these scars. Be sure to see a dermatologist for treatment if you get acne early — between 8 and 12 years old. If someone in your family had acne cysts and nodules, you also should see a dermatologist if you get acne. Treating acne before cysts and nodules appear can prevent scars.
DEPRESSION AND ACNE SCARS
DIAGNOSIS: ACNE STAGES
Acne patients are often treated by family doctors. Complicated cases are referred to a dermatologist, a skin specialist, or an endocrinologist, a specialist who treats diseases of the body's endocrine (hormones and glands) system. To diagnose acne, a dermatologist will first examine your skin to make sure you have acne. Other skin conditions can look like acne. If you have acne, the dermatologist will: 
  • Grade the acne. Grade 1 is mild acne. Grade 4 is severe acne. 
  • Note what type, or types of acne appear on your skin.
In cases of minor-to-moderate acne, a person will generally have to use home and over-the-counter remedies consistently for 4–8 weeks before they see results. More severe inflammatory types of acne tend to take much longer to clear up. Speak and consult a doctor or dermatologist if  whiteheads, blackheads, papules, or pustules: 
  • are severe, very painful and large 
  • do not respond to over-the-counter medications 
  • bleed and release a lot of pus 
  • cover a significant portion of the face or body
  • cause emotional distress 
  • develop very close to sensitive areas, such as the eyes or lips 
Acne treatment consists of reducing sebum production, removing dead skin cells, and killing bacteria with topical drugs and oral medications. Treatment choice depends upon whether the acne is mild, moderate, or severe. Receiving prompt, correct treatment can reduce the risk of long-term skin complications, such as dark spots and scarring.
ACNE SCARS TYPE
Acne scars fall into two main categories: those caused by a loss of tissue (atrophic scars), and those caused by an excess of tissue (hypertrophic scars). Within these categories, there are four main types of acne scars: ice pick, boxcar, rolling, and keloid scars.

It is important for family physicians to educate patients about available treatment options and their expected outcomes. Acne scars are stubborn, and no single treatment is best for everyone. Various methods may help improve your complexion. Your doctor may suggest one or a combination of these:

Acne is not curable, but can be controlled by proper treatment, with improvement taking two or more months. Acne tends to reappear when treatment stops, but spontaneously improves over time.
